The Quiet Epidemic Hidden in Plain Sight

Every doctor has patients who never appear to have a drug problem.

There is the physiotherapist who keeps one Valium tablet for Saturday nights—not because she is in crisis, but because knowing it is there makes the week feel manageable.

There is the young executive who carries a strip of Ativan in his wallet as an “exit policy” against unbearable anxiety.

Neither considers themselves addicted. Both received legitimate prescriptions. Both represent a much larger public health issue that remains largely invisible.

Benzodiazepines—commonly known as benzos—including diazepam (Valium), lorazepam (Ativan), alprazolam (Xanax), clonazepam, and others, have transformed the treatment of anxiety, insomnia, seizures, muscle spasm, and alcohol withdrawal since their introduction in the 1960s. Used appropriately, they are among the most effective medicines in modern medicine. Used beyond their intended role, however, they can quietly reshape the brain, creating dependence that is often mistaken for the original illness.

How They Work

The human brain maintains a delicate balance between excitation and inhibition.

Gamma-aminobutyric acid (GABA) is the brain’s principal inhibitory neurotransmitter. It acts as the nervous system’s natural brake, calming excessive neuronal activity.

Benzodiazepines do not create calm by themselves. Instead, they amplify GABA’s effect by binding to a specific site on the GABA-A receptor, increasing the frequency with which chloride channels open. The result is reduced neuronal firing throughout the central nervous system.

Clinically, this produces familiar effects:

  • reduced anxiety
  • sedation and sleep
  • muscle relaxation
  • anticonvulsant activity
  • temporary amnesia during medical procedures

The relief is often rapid, sometimes within minutes. That speed is precisely why these medicines are so valued—and why they can become so difficult to leave behind.

When Treatment Becomes Dependence

Most people associate dependence with misuse or illicit drug use.

The reality is often very different.

Many patients develop dependence while taking benzodiazepines exactly as prescribed.

This is known as iatrogenic dependence—harm resulting from medical treatment itself.

As the brain adapts to repeated exposure, GABA receptors become less responsive while excitatory systems compensate. Over time, the original dose becomes less effective, a process known as tolerance.

The patient may believe their anxiety has worsened.

In reality, the brain may simply have adapted to the medication.

This distinction is crucial because escalating the dose often treats tolerance rather than the underlying condition.

The Withdrawal That Looks Like Relapse

Stopping benzodiazepines abruptly can be medically dangerous.

Withdrawal may begin within hours or days, depending on the medicine’s half-life.

Symptoms commonly include:

  • intense rebound anxiety
  • insomnia
  • tremor
  • sweating
  • heightened sensitivity to sound, light, and touch
  • palpitations
  • panic attacks

More severe cases can involve hallucinations, delirium, or seizures.

For some patients, recovery extends well beyond the expected withdrawal period. Persistent neurological symptoms lasting months—or occasionally years—have increasingly been recognised as Benzodiazepine-Induced Neurological Dysfunction (BIND). These individuals may experience cognitive impairment, fatigue, tinnitus, emotional blunting, poor concentration, and persistent anxiety long after the medication has been discontinued.

Repeated cycles of stopping and restarting can make subsequent withdrawals progressively more severe, a phenomenon known as kindling.

Why Long-Term Use Often Fails

Benzodiazepines were never intended as lifelong treatment for anxiety or insomnia.

Most clinical guidelines recommend limiting their routine use to two to four weeks except in carefully selected situations.

Although patients often continue treatment for months or years, the benefits frequently diminish while risks steadily accumulate.

Long-term use has been associated with:

  • impaired memory and concentration
  • slower cognitive processing
  • increased falls and fractures, particularly in older adults
  • emotional flattening
  • worsening depression in susceptible individuals
  • persistent physical dependence

Whether benzodiazepines directly increase dementia risk remains debated, but the cognitive effects in older adults are sufficiently well established that prolonged prescribing requires careful justification.

Ironically, medicines initially prescribed for anxiety or poor sleep may eventually perpetuate both conditions.

The Human Story Behind the Prescription

Dependence rarely begins with recklessness.

It begins with trust.

A patient follows medical advice.

A prescription is renewed because life remains stressful.

Months become years.

Eventually, the medicine is no longer taken to feel better but to avoid feeling dramatically worse.

This is not a moral failure.

It is a predictable consequence of neuroadaptation.

Understanding that distinction reduces stigma while encouraging more thoughtful prescribing.

The Indian Context

In India, benzodiazepines are regulated as Schedule H and Schedule H1 medicines and legally require a prescription.

Yet in many parts of the country they remain readily obtainable without one, reflecting inconsistent enforcement rather than a lack of regulation.

Easy access normalises long-term use, encourages self-medication, and delays recognition of dependence. Public awareness, pharmacist education, and responsible prescribing are all essential if these medicines are to retain their legitimate clinical role without becoming a hidden source of harm.

Beyond Medication

None of this suggests benzodiazepines should never be prescribed.

For panic attacks, status epilepticus, alcohol withdrawal, procedural sedation, and selected short-term crises, they remain indispensable.

The challenge is ensuring that short-term treatment does not quietly become long-term dependence.

For chronic anxiety and insomnia, evidence increasingly supports psychological therapies, cognitive behavioural therapy, sleep interventions, exercise, stress management, and treatment of underlying psychiatric or medical conditions as safer long-term strategies.

World Benzodiazepine Awareness Day

Each year on 11 July, World Benzodiazepine Awareness Day commemorates the birthday of Professor Heather Ashton, whose pioneering work transformed understanding of benzodiazepine dependence and withdrawal.

The movement is not an anti-medication campaign.

Its central message is simple:

Patients deserve informed consent before treatment begins, careful monitoring during treatment, and compassionate, evidence-based support if discontinuation becomes necessary.

The goal is neither fear nor prohibition.

It is wiser prescribing, better education, and recognition that medicines capable of relieving suffering can also create it when their risks are underestimated.

Awareness is not about discouraging appropriate use.

It is about ensuring that every prescription is accompanied by the knowledge needed to use these powerful medicines safely, for the shortest duration necessary, and with a clear plan for review and, when appropriate, gradual discontinuation under medical supervision.
